Introducting New OKU Card by Welfare

Re: INTRODUCTING NEW OKU CARD - I.C.-SIZE (Replacing the Existing OKU Card)

Please be inform that Jabatan Kebajikan Masyarakat (JKM) has Introduce a New OKU Card (IC size) to replace the Existing OKU Card.

COME EVERYONE!! and Replace your Existing OKU Card at the Welfare Booth on the 1st Floor at YMCA of Kuala Lumpur.

Below are the details:-

Date       :  11th September 2011 (Sunday)

Time       :  9.00am to 4.00pm

Venue     :  H1, H2 and H3,  YMCA KL

For Who  : OKU - Hearing Impairment

1) For those who have the Existing OKU card and want to apply for New OKU card (IC size)

    PLEASE WEAR LIGHT COLOUR BLOUSE/SHIRT  

2) For those who have not yet register with the Welfare for OKU card 

    PLEASE WEAR LIGHT COLOUR BLOUSE/SHIRT and please bring - 1 photocopy of MYKAD for registration.
